Consent to Do Business Electronically & to Access
Information Electronically (“Consent”)

What is the purpose of this Consent?

For certain kinds of requests and services related to an insurance policy (“Policy”) from Transamerica Life Insurance Company, Transamerica Financial Life Insurance Company, or Monumental Life Insurance Company (the “Company”), you have expressed your desire to conduct business electronically. To conduct business electronically and/or access information electronically, you must provide the Company, and its authorized designees and agents, with your consent. By accepting the terms of this Consent, you will be providing the Company and its authorized designees and agents, with your consent:

  1. To have information relating to the Policy made available to you electronically;
  2. To submit, via electronic means, requests relating to the Policy; and
  3. To all of the terms and conditions set forth in this Consent.

This Consent covers your agreement as policy owner or certificate holder to be bound with the same force and effect as if you had signed your name on paper by hand when you submit requests relating to the Policy. You understand that by entering your user name and password and making a request, or otherwise following a procedure to verify your identify and intent to make a request, that you are applying your electronic signature to your request. And further, you will maintain the security of your user name, password, or other information needed to apply your electronic signature.

What kinds of transactions may be conducted electronically?

In addition to making information relating to the Policy available to you electronically, this Consent applies to any requests you make electronically relating to the Policy as policy owner or certificate holder, including but not limited to the following requests, as they are available: requests for change of address or change of other contact information, requests for forms relating to specific transactions or claims, requests to change ownership or ownership information, requests to change beneficiaries, requests relating to premium payments, requests for annual statements, illustrations or other information, requests for withdrawals, requests for insurance benefits, and requests for surrender or cancellation of the Policy or a certificate of coverage.

Even though you have provided the Company with this Consent, the Company may, at its option: (a) deliver documents and information to you on paper, and (b) require that certain communications from you be delivered to the Company on paper.

If I prefer to use paper instead of conducting a transaction electronically, may I use paper?

Yes. You may obtain and use paper copies of any forms to transact business relating to the Policy at any time and without charge by contacting the Company at the address provided below. If you do not wish to access forms or documents electronically, please call the Company’s Customer Service Department at (888) 763-7474 or TransConnect Customer Service (866) 224-3100 to assist you.

Should I maintain copies of forms and documents?

Yes. You agree to print or save this Consent and all forms and documents sent or made available to you electronically, and to keep printed or electronic copies of them for your records. If you have any trouble with printing or saving, you should contact the Company.

How long will this Consent remain in effect?

This Consent shall become effective once you accept the terms of this Consent, and remains in effect for so long as the Policy remains in effect, or until you withdraw your consent (as described in the next section), whichever occurs first. This Consent is subject to change, update and/or revision (collectively "change") by the Company in its sole discretion at any time, without notice. Your use of this website after any such change constitutes your acknowledgment and acceptance of such change. You should therefore periodically visit this page to review the most current Consent.

What if I change my mind?

If at any time you would like to cease doing business electronically with the Company with respect to the Policy, you will need to provide the Company with written notice of your withdrawal of consent to do business electronically, which will then terminate this Consent. You may withdraw consent at any time and without charge by contacting the companies. Your withdrawal of consent and the termination of this Consent will become effective two (2) business days after the Company receives your written notice of withdrawal. Thereafter, all forms, documents and information will be provided to you on paper.

What if my contact information changes?

If you are the owner of the Policy or certificate of coverage, you must keep the Company informed of any changes to your email address(es) and all other contact information by contacting the Company at the contact information provided below. You agree to hold the Company harmless with respect to any emails sent to the incorrect email address due to your failure to provide the Company with a current or valid email address.

What equipment do I need to do business electronically with the Company?

To access and retain forms, documents and information made available to you electronically by the Company, you must have access to a computer with an Internet connection. You must be able to send and receive emails, and be able to save forms and documents to a storage device for later reference or have the computer connected to a printer so you can print out such documents. Unless notified otherwise, the Company may provide or make these documents available to your agents and insurance representatives
